text-align: left;\"><b><i>Did you know?<\/i><\/b><\/h5>\n<p style=\"padding-left: 40px; text-align: justify;\"><i><span style=\"font-weight: 400;\">This sciatica has been around since the time of the Ancient Greeks in 370BC!<\/span><\/i><i><span style=\"font-weight: 400;\"><br \/>\n<\/span><\/i><i><span style=\"font-weight: 400;\">Hippocrates described \u2018ischias\u2019 &#8211; pain felt around the hip and thigh &#8211; affecting men around 40 to 60 years old, and that young men who felt it would often have pain lasting around 40 days before resolving spontaneously!<br \/>\n<\/span><\/i><i><span style=\"font-weight: 400;\"><br \/>\n<\/span><\/i><i><span style=\"font-weight: 400;\">From the 17th century all the way till the 19th century, physicians knew of sciatica but were unsure about what caused it &#8211;<\/span><\/i><\/p>\n<p style=\"padding-left: 40px; text-align: justify;\"><i><span style=\"font-weight: 400;\">Until the 20th century. <\/span><\/i><i><span style=\"font-weight: 400;\"><br \/>\n<\/span><\/i><i><span style=\"font-weight: 400;\">In 1931, a neurosurgeon named Elsberg described that the removal of \u2018cartilaginous tumours\u2019 from the spinal canal improved sciatic symptoms; and 3 years later in 1934, a famous paper by the New England Journal of Medicine cemented the idea that the prolapsed intervertebral disc caused sciatica.<\/span><\/i><\/p>\n<p style=\"text-align: justify;\">&nbsp;<\/p>\n<h4 style=\"text-align: left;\"><b>\u2026 Is it really though?\u2026<\/b><\/h4>\n<p style=\"text-align: justify;\"><span style=\"font-weight: 400;\">The most commonly blamed cause of sciatica is this:&nbsp;<\/span><\/p>\n<ul style=\"text-align: justify;\">\n<li><span style=\"font-weight: 400;\">A herniated lumbar disc where the nerve root is compressed by disc material that has ruptured through its surrounding annulus.<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Lumbar spine degeneration causing narrowing of the foraminal spaces, again causes nerve root compression.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><\/li>\n<\/ul>\n<p><a href=\"https:\/\/spinefitchiro.com\/wp-content\/uploads\/2020\/07\/disc-1080x630-1.jpg\"><img decoding=\"async\" class=\"aligncenter wp-image-23530 size-large\" src=\"https:\/\/spinefitchiro.com\/wp-content\/uploads\/2020\/07\/disc-1080x630-1-1024x597.jpg\" alt=\"Sciatica treatment Kuala Lumpur back and leg pain\" width=\"680\" height=\"396\" srcset=\"https:\/\/spinefitchiro.com\/wp-content\/uploads\/2020\/07\/disc-1080x630-1-1024x597.jpg 1024w, https:\/\/spinefitchiro.com\/wp-content\/uploads\/2020\/07\/disc-1080x630-1-300x175.jpg 300w, https:\/\/spinefitchiro.com\/wp-content\/uploads\/2020\/07\/disc-1080x630-1-768x448.jpg 768w, https:\/\/spinefitchiro.com\/wp-content\/uploads\/2020\/07\/disc-1080x630-1.jpg 1080w\" sizes=\"(max-width: 680px) 100vw, 680px\" \/><\/a><\/p>\n<p style=\"text-align: justify;\">&nbsp;<\/p>\n<p style=\"text-align: justify;\"><span style=\"font-weight: 400;\">We NOW know that imaging scans (like MRIs) show that <\/span><span style=\"font-weight: 400;\">disc pathology (disc bulges and herniations), and stenosis with neural compromise (when the lumbar spine degenerates and causes narrowing of the tunnel where the nerve comes out from) is actually a relatively common finding in people WITHOUT any symptoms of sciatica, or back pain! (Brinjikji et al., 2015; Boos et al., 2000).<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-weight: 400;\">Patients with pain AND a disc herniation seen on MRI may experience <\/span><span style=\"font-weight: 400;\">marked improvement in symptoms WITHOUT any changes in your spine or disc, whereas the removal of herniated disc material or other causes of nerve root compression DOES NOT ALWAYS relieve pain.<\/span><\/p>\n<p style=\"text-align: justify;\">&nbsp;<\/p>\n<h4 style=\"text-align: left;\"><b>So why does sciatica actually happen?<\/b><\/h4>\n<p style=\"text-align: justify;\"><span style=\"font-weight: 400;\">Evidence suggests that it is not so much just pressure on the nerve root that causes sciatica, but a COMBINATION of immunological responses in the body &#8211; inflamed, stretched, and\/or compressed nerve roots causes sciatic pain. <\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">This CAN be because of an injury causing a disc bulge and pressure on the nerves, a <a href=\"https:\/\/spinefitchiro.com\/services\/chiropractic\/\">muscle spasm<\/a> that causes temporary pinching of the nerve, and other lifestyle factors which increase inflammation in the body like excessive alcohol, smoking, poor diet, inadequate sleep, and mental stress.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-weight: 400;\">That being said, having a \u201cdiagnosis\u201d of \u201cSciatica\u201d is JUST identifying a symptom.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-weight: 400;\">There needs to be an in-person <a href=\"https:\/\/spinefitchiro.com\/initial-visits\/\">assessment<\/a> at our Mont Kiara practice of your complete history, and a physical consultation to identify what activities may need to be modified for the time being to help your <a href=\"https:\/\/spinefitchiro.com\/services\/physiotherapy-and-rehabilitation-treatment-malaysia\/\">active recovery<\/a>. Slouch all the way down, as much as you can, including the head and neck. <\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">(b) Keeping this position, slowly attempt to straighten out the right knee and bring your foot towards you. Take note if you feel any pain or not.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">(c) Keeping this position, raise the head and look up. Did the pain get better, or did it remain?<br \/>\n<\/span><span style=\"font-weight: 400;\">(d) Put the leg down and repeat steps (a) to (c) on the opposite leg.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"><br \/>\nIf the pain gets better when you look up, this indicates a disc and nerve problem. If it doesn\u2019t, this indicates a muscle issue.<\/span><\/td>\n<td style=\"width: 33.333333333333336%; border-style: solid; border-color: #000000; vertical-align: top;\">\n<h5 dir=\"ltr\"><strong>McKenzie\u2019s Repeated Test<\/strong><\/h5>\n<p dir=\"ltr\">(a) Bend forward slowly, attempting to touch your toes, slowly come back up, and repeat for 10 times. Does your pain get worse as you repeat?<\/p>\n<p dir=\"ltr\">(b) Now put your hands on your hips, and slowly bend backwards 10 times. Does your pain get better?<\/p>\n<p>\u30fbPain that gets better as you repeatedly bend backwards could indicate a disc&nbsp; issue.<br \/>\n\u30fbPain that doesn\u2019t get better, or gets worse, may indicate a muscle spasm.<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p style=\"text-align: justify;\">&nbsp;<\/p>\n<h4 style=\"text-align: left;\"><b>How long will it take to go off?<\/b><\/h4>\n<p style=\"text-align: justify;\"><span style=\"font-weight: 400;\">Without the presence of red flags, treatment of sciatica in the first 6\u20138 weeks should take a <a href=\"https:\/\/spinefitchiro.com\/chiropractic-and-physiotherapy-combined\/\">conservative approach<\/a>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-weight: 400;\">In patients with severe symptoms who fail to respond following 6\u20138 weeks of non-surgical treatment, imaging might be useful to identify if a herniated disc with nerve root compression is present. Surgery may only be indicated if imaging findings correspond well with the clinical symptoms, there was no success with conservative care, and\/or the patient develops red flags.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-weight: 400;\">WITHOUT surgery, 80% of the patients recover within 8 weeks and 95% within 1 year.
